Disorder of bone turnover- excessive bone turnover
What is excessive bone turnover due to?
Excessive osteoclast & osteoblast activity
What does Paget’s disease of bone result in?
Enlarged misshapen bones + structural problems that increase risk of pathological fractures
What part of skeleton does Paget’s particularly affect?
Axial

- Bone pain
- Bone deformity

- Hearing loss can occur
X-ray findings in Paget’s?
- Bone enlargement & deformity
- Osteoporosis circumscripta
- Cotton wool appearance of skull
- V-shaped defects in long bones

- Raised alkaline phosphatase (ALP) + other LFTs normal

Main treatment of Paget’s
Bisphosphonates
Other treatment options for Paget’s?
NSAIDs for bone pain
Ca + Vit D supplementation
Surgery (rarely required)
What do you continuously monitor when someone has Paget’s?
Serum Alkaline phosphatase

What is osteoporosis circumscripta?
Well defined osteolytic lesions that appear less dense compared with normal bone
Complications of Paget’s disease?
Osteogenic sarcoma (osteosarcoma) Spinal stenosis
What is osteosarcoma?
Presents with
- Increased focal pain
- Bone swelling
- Pathological fractures
What is spinal stenosis?
Deformity in spine leads to spinal cord narrowing
