How do chemical methods cause lice death
Paralysis and dehydration
What is the only ovicidal drug we discussed?
Malathion
Malathion MOA
Lice metabolize it to malaoxon which inhibits AChE, causing paralysis
Malathion AEs
In humans, converted quickly to inactive metal - NOT assoc with cholinergic toxicity unless ingested - GI peristalsis, less ocular accommodation, wheezing, salivation, sweating, hypotension, musculary/resp paralysis
What is the treatment to reverse malathion overdose?
Atropine and pralidoxime
Permethrin MOA
Bind voltage-gated Na channels leading to paralysis
Permethrin AEs
Safe bc human channels not susceptible; rare AE is asthma exacerbation in those allergic to ragweed
Permethrin efficacy issues
Decreased due to kdr mut in louse Na channels
Lindane issues
disfavored b/c of potentially neurotoxic and remains in environment
When do you give patients lindane
Failure of first-line therapy; seizures reported with multiple applications (CNS stimulant)
Lindane AEs
CNS stimulant through GABA blockade
Lindane BBWs?
Skin diseases, neonatal prematurity-inc drug systematization
C/I uncontrolled seizures (Crazy seizing Linda can’t take lindane)
Ivermectin when to give
NOT to kids! Crosses BBB
Adults only because doesn’t X BBB
