Colchicine-Indications
Reduces frequency and pain of acute gout attacks. Must be administered within 24-48 hrs
Colchicine- MOA
Decreases migration of granulocytes, blocks cell division and inhibits release and synthesis of leukotrienes
Colchicine- AE
N/V/ abd pain, diarrhea, neutropneia, aplastic anemia
Xanthine oxidase inhibitors- (Allopurinol)- Indications
Primary hyperuricemia of gout and hyperuricemia secondary to other conditions
Xanthine oxidase inhibitors- (Allopurinol)- MOA
Competitively inhibits uric acid biosynthesis
Xanthine oxidase inhibitors- (Allopurinol)- AE
Hypersensativity reactions, GI
Ucosurics (Probenecid)- Indications
Gout
Ucosurics (Probenecid)- MOA
Promotes renal clearance of uric acid- inhibits anion exchanger in proximal tubule
Ucosurics (Probenecid)- AE
HA/N/V anemia, dizziness
Renal function & gout
Maintain serum urate at 6.0mg/dL or less- precipitated crystals will dissolve and clear.
6.8mg/dL and above will only slow progression.
Allopurinol agent of choice
