What is person centred care?

A

Care that focuses on the needs of the individual where people’s preferences, needs and values are considered. The care needs to be respectful and responsive to the patient, where health and wellbeing outcomes are co-produced

A

It allows us to see the personal situations of patients. E.g. when you see a patient in a wheelchair- can they walk? Will they fall off the x-ray table? Talk to the patient to gain more information about their situation

Many want to play more of an active role in their healthcare

Supporting patients with long term health conditions can improve their clinical outcomes

Collaborating with the patient and other healthcare professionals means that people will use less emergency hospital services because their issues will have been fully resolves

People are more satisfied with their care

Patients are more likely to follow the plan and take their medications regularly

Find a common ground- have you watched the same movie? Or read the same book? Relate to them

Introduce yourself

Let them know what’s happening

Don’t overload them with information. Tell them what they need to know at each stage- not all at once

Build relationships with patients to instantly gain their trust
