How can you tell if the patient has a prominent frenum?

A
  • When the frenum placed under tension the incisive papilla blanches
  • radiographically there is a notch seen on the crest of the interdental bone between the upper teeth
  • The anterior teeth may be crowded

Physiological (normal)

How would you manage a prominent frenum?

A
  1. take a PA to exclude the presence of a supernumerary
  2. in developing dentition a diastema of less than 3mm does not warrant any treatment
  3. I’m developing dentition if diastems more than 3mm and lateral present then could consider an appliance to approximate the centrals providing enough space for laterals and canines to erupt: need to be careful roots do not impeded on any crowns
  4. Distally tilted crowns can consider using a removable appliance but if you need full bodily movement then need fixed
  5. Needs long term retention!!!

Format? 2 components: DHC and AC, average score can be taken across the two but DHC usually used alone

A

Developed: based on am index used in Sweden
Purpose: reflect occlusal traits affecting function and longevity of dentition
How? Single worst feature noted and then categorised as being 1/2/3/4/5 (1:no need 5: v great need). Can use MOCDO to help determine which is the single worst feature

Problems: subjective, nothing to represent class 3 or AOB
