Interceptive extractions of permanent teeth Flashcards
When do we perform interceptive extractions of permanent teeth?
To guide or permit the eruption of teeth
to relive crowding
How often is serial extractions of teeth performed?
Not often
popular in the past
What was the main hope when performing serial extractions?
transfer incisor crowding to 3 region then cure this by extracting 4’s
extraction sequence was C’s, D’s and 4’s
In which cases were serial extractions most useful in?
class 1
What are the problems with extracting 6’s as part of orthodontic treatment?
makes orthodontic treatment more difficult because the space created is very far from the the front were most problems usually exist
What are the indicators for 6’s with poor prognosis?
Gross caries hypoplasia large restorations recurrent carie lingual/buccal caries
What is the success of interceptive extractions dependant upon?
Timing
Crowding
Tooth extracted (upper or lower 6)
Type of malocclusion
At what age should the FPM be extracted to achieve optimal space closure between 5 and 7?
When the bifurcation of the lower 7 is forming
What happens if the 6 is extracted too early?
the 5 may escape distally and impact against the 7
What happens if the 6 is extracted too late?
the space closure will be incomplete and the 7 tips into place rather than moving mesially
When is it better to extract 6’s, too early or too late? and why?
too early
premolars are less likely to tip distally but greater chance of 7 tipping mesially
more difficult to close a space posteriroly due to lack of anchorage
under which circumstances may you consider extracting the 6’s ealier?
premolar crowding
7 is ditoangular
If there is already evidence of spacing within the arches, what happens once the FPM are extracted?
there will be no spontaneous space closure
Which molars do better following interceptive extractions?
Upper molars perform better because the teeth in the upper arch diverge and lower arch converge
What is balancing?
When the same tooth is taken out on the same arch
