What is the most common location for parosteal osteosarcoma?
A
Posterior distal femur, proximal tibia, proximal humerus
80% occur in the femur

What are some imaging and histological characteristics of parosteal osteosarcoma?
A
1) Heavily ossified, lobulated mass with “stuck on” appearance on x-ray
2) Hot on bone scan
3) CT for staging; MRI to see extent of soft tissue involvement
4) Atypical spindle cells within normal trabeculae; has low grade appearance
5) Can have similar appearance to fibrous dysplasia, but different radiographically

What is the treatment for parosteal osteosarcoma?
A
Wide local excision; adjunct chemo only indicated in high grade lesions
