A
- KAFO medial upright too high
- insufficient shoe lift
- short leg
- Hip pain; weak hip abductors

A
- Locked knee
- Excessive plantar flexion (inadequate stop, plantar flexion contractures)
- Weak hip flexors or dorsiflexors
Any of these can lead to vaulting as well
