CHF Systolic Dysfunction
Mechanism of Action of Digoxin?
- Slows HR
- Increased Strength of Contraction
Forms fewer but better contractions
(+) Inotropic Drug
ECG changes associated with Digitalis?
Prolonged PR interval and a Shortened QT interval
+ ST segment depression
Digoxin Toxicity Effects
- Arrythmias
- Palpitations
- Fatigue
- Confusion and Delirium
- Hyperkalemia
How often is Digitalis used?
Used as a short term drug, never should be used for maintenance
Effects of Nitroglycerine (nitrates)
Vasodilation of arterial and venous vessels
- Can lead to an increase in HR via Reflexive Tachycardia
Main indication for the use of Nitrates?
Angina Pectoris
How to deliver Nitrates? How often in the even of Angina?
Sublingually
3 dose max, 1 does every 5 minutes and see effects.
When should EMS be called with taking nitrates for angina?
- If after the first dose there is increase in symptoms
- If after 3 doses there is no change, patient may be having an MI
Patients position for taking Nitrates?
Sitting or Supine never in standing as they can pass TF out
Side Effects of Nitrates
- Reflexive Tachycardia
- Venous Pooling
- Dizziness
- Lightheadedness
- OH
- Headache
Patients should take what type of drugs for the prevention of angina long term?
Beta Blockers or Calcium Channel Blockers
Affects of Nitrates on Preload and Afterload?
Lowers both preload and Afterload
