Obsessions
recurrent, persistent, intrusive, and unwanted thoughts, images, or urges that cause anxiety or distress
Compulsions
Repetitive behaviours or thoughts that a person engages in to neutralise, counteract or make their obsession go away
Symptom dimensions
Contamination
Responsibility for causing/not preventing harm
Taboo thoughts about sex, violence and blasphemy
Need for order and symmetry
Specifiers DSM
With good or fair insight; knowing its not true
With poor insight; thinks they are true
With absent insight/delusional beliefs; completely convinced they’re true
Tic-related; current or past of tics
Ego
Self-image that forms the bridge between the conscious and unconscious thoughts
Egosyntonic
Thoughts are perceived as their own. Responsive and appropriate to the environment given the situation
Egodystonic
Thoughts are not theirs. Response and behaviours are against a persons believes and will. Not perceived as their own.
Differentiation OCD vs OCPD
OCD: Obsessions and/or compulsions causing anxiety/distress. Egodystonic on one or a few domains.
OCPD: personality disorder with need for control/perfectionism/rigidity. Tension, egosynotonic and on multiple domains.
Comorbidity OCD and OCPD + Prev OCD
Comorbidity: 23%. Makes differentiating difficult.
OCD; prev is 1-3 %
Around 25% onset before 10 yrs
Around 60% onset around adolescence
Chronic relapsing course. Stress is the fertiliser of OCD
