What is a Prolapsed Cord?
Cord is out and baby is still in. Chances are baby is pushing on it,
cutting off life support for baby.
What is the care for a prolapsed cord?
- Put mother face down, pelvis up
- High Concentration Oxygen
- Insert hand and lift baby off of cord
What is Limb Presentation?
Baby’s arm or leg has dropped quicker than rest of body. Presenting itself individually.
What is the care for Limb Presentation?
- Transport
- Mother head down, pelvis up
- Do not pull or replace the limb
- Only Put your hands in if prolapsed cord
- Give mother high conc o2
- Notify hospital
What can blood at the meatus indicate?
urethral damage or a pelvic injury
What is the Placenta?
The organ of pregnancy, where exchange of oxygen, nutrients, and waste occurs between mother and fetus.
What is the Umbilical Cord?
the fetal structure containing the blood vessels that carry blood to and from placenta.
What is the Amniotic Sac?
bag of fluids surrounding the developing fetus
What are the 3 stages of delivery?
- regular contractions, thinning and gradual dilation of cervix. ends when cervix is fully dilated
- stage when baby enters birth canal and lasts till baby is born.
- after baby is born till afterbirth is all delivered.
How do you control bleeding after birth?
- Pressure
- lower the legs
- fundal massage
- nursing.
What are the proper steps for prepping a mother for delivery?
- control scene
- ppe
- mother on bed, butt elevated with pillow, knees up and drawn apart
- remove clothing
- have someone at head of mother
- position kit near patient and have easy access
- warm ambulance
Describe the steps to assuring mother with a normal birth…
- Partner at head - vitals and monitoring mother
- position gloved hand @ vaginal opening, support babies head to prevent explosive birth
- place one hand below head, against perineum, apply pressure to prevent ripping
- If Amniotic sac is not punctured yet - rip with fingernails or use scalpel. pull away from babies nose and mouth, support infants head, check for cord, aid in birth of upper shoulder, support trunk, pelvis and lower extremity. Keep infant level with vagina until umbilical cord stops pulsating.
- Assess Airway - APGAR
- Note time of Birth
What are the questions to ask a woman in Labor?
- when is your due date?
- is this your first pregnancy?
- are you under the care of a physician?
- when did labor pains start?
- has your water broke? did it smell? was there blood?
- Do you feel like you need to push?
- do you feel the urge to poop?
- may i check for crowning?
- may i take your vitals?
Recite the steps for a neonatal resuscitation assessment…
- Is Neonate Breathing? Is Neonate Active?
No - Intervene (BVM)
Yes - Dry, Warm, Stimulate (30 seconds) - Is Breathing absent, or gasp like? Is heart rate less than 100 bpm?
Yes - PPV with BVM (30 seconds)
No - Step 3 - Reassess Heart Rate
Less 100 - PPV (30 seconds)
Less 60 - Begin Compressions (30)
Recite the APGAR scoring chart…
APPEARANCE-
0- blue all over
1- extremities blue, trunk pink
2- pink all over
PULSE -
0 - 0
1 - less than 100bpm
2 - over 100bpm
GRIMACE -
0 - no reaction
1 - facial
2 - sneeze/cough/cry
ACTIVITY -
0 - none
1 - only slight
2 - normal
RESPIRATORY -
0 - none
1 - slow/ irregular, weak crying
2 - good breathing, strong cry
