What are the 4 P’s of Musculoskeletal?
- Paresthesia- Pins/Needles
- Pulses - Diminished/Absent
- Paralysis - Inability to move
- Pressure
Musculoskeletal Injury Assessment
-Pain/Tenderness
-Deformity/Angulation
-Grating/Crepitous
-Swelling
-Bruising
-Exposed Bone
-Locked Joint
-Nerve + Blood Vessels Compromise
Realignment of Deformed Extremity, the process
-One EMR grabs distal extremity while EMR2 places one hand above and below the injury site
-EMR performs manual reactions to realign bone
-If there is resistance, just splint
Signs of an Unaligned Bone?
-splint will be inaffective
-compromised nerves, arteries, veins
-increased pain
What is the care for a Rib Fracture?
- Consider ALS
- Position of Comfort if no Cspine
- Oxygen
- Hold pillow to chest
What is the Care for a Hip Fracture?
- ALS
- Bind Legs together with padded boards between legs.
- Oxygen
- Transport.
What are the signs and symptoms of a Hip Fracture?
-pain is localized
-sensitive to touch
-discolouration
-swelling
-unable to move limb while on back
-cannot stand
-rotation of foot
-injured limb shorter
What will you see in a shoulder girdle injury?
-pain in shoulder
-dropped shoulder
-severe blunt force trauma to back
Shoulder Girdle Injury Care
- Assess CSM - if impaired…
•Immobilize and transport
-use sling to immobilize
-if evidence of dislocation…place pillow between arm and torso
-do not attempt to straighten
-Assess CSM
What will you see with a knee injury?
-pain and tenderness
-swelling
-deformity w obvious swelling
What are the steps of care for a bent knee injury?
- Assess CSM
- Immobilize knee
- Tie 2 padded bars splints
- use pillow to stabilize
- CSM
What are the two types of traction splints?
-Bipolar
-unipolar
What is a bipolar traction splint?
Splint that cradles leg between 2 metal boards.
What is a unipolar leg splint?
a splint used with one rod placed alongside leg.
What is a vacuum splint?
A splint with air sucked out of it to tighten.
What is the Care for Pelvis injuries?
-move patient as little as possible
-Determine CSM
-straighten lower limbs into a anatomical position.
-Stabilize lower limbs to reduce further injury (use cravats on upper thigh, above/below knee, and ankles)
-assume spinal injuries and immobilize.
-CSM
-care for shock
-transport and monitor.
What are the Steps of Pelvic Stabilization? (sheets not pelvic binder)
- Prepare backboard, and lay a sheet where the hips will be laying.
- 2/3 man log roll patient onto back board.
- Tightly bring sheets around front of patient, without moving hips out of place. Tie to secure.
- Secure knots with clamps.
Indications for forearm, wrist, hand splinting
•Forearm - deformity, tenderness
•Wrist - deformity, tenderness
•Hand - deformity, pain
What is the Care for Elbow injuries (straight position)?
- CSM
- Use a Padded Board Splint, be sure to pad the armpit
- Curl fingers around the end of splint
- Secure with roll gauze above and below injury
- Pad in between the splint and the body (pillow)
- Secure with cravats across the body
- Reassess CSM
What is the Care for a Bent Elbow Injury?
- Check CSM
- Use a sling, keep the elbow bent
- Secure sling with a swathe
- CSM
How to Apply a Sling/Swathe?
- Fold cloth cravat into a triangle
- Position over chest, place folded arm over cravat
- Extend over injury, pull up and over injured side, bring bottom piece behind good shoulder.
- Draw up elbow and tie, ensure the knot is in a comfortable place and pad.
- Expose fingertips, check CSM
- Use cravat as swathe, tie around injury and chest, secure
- CSM
