antihistamines, nasal steroids, topical cromolyn, leukotriene modifier
Sinusitis bacterial vs viral time
7-10 days for viral infection to resolve
7-10+ days=bacterial
double sickening, tooth pain, pain leaning forward
sinusitis organisms (3)
strep pna, h. flu, m. cat
sinusitis dx ct only needed w
recurrent sinusitis/resistant

amoxicilin-clavulanate 1st line
3rd gen cephalosporins
doxy, levofloxacin, moxifloxacin if PCN allergy
PO steroids
When to refer sinusitis
xResponse w 3-4 wk abx, nasal polyps, periorbital cellulitis (Emergenc)
Epistaxis anterior vs posterior
anterior-kiesselbach plexus
posterior-sphenopalatine ARTERIES, asymptomatic, nausea, hematemesis, anemia, hemoptysis, melena
espistaxis causes
sinusitis, rhinitis neoplasm/polyps, smoke, corticosteroids, septal deviation, telangiectasia
Epistaxis tx
Phenylephrine
Cautery w silver nitrate stick if visible bleed
