Patho CAD
atherosclerosis- lipid deposits/interluminal thickening=atheromatous plaque
angina meds
nitro, isosorbide,b blockers (^activity, vO2 demand,^o2 supply), Ca channel blocker, ranolazine (^activity,^bp)
chronic angina guidelines
antiplt,cholesterol drugs, ACEi, antianginal
Vasospastic angina tx
whats not effective
Ca channel blocker oR Long acting nitrate, if inadequate add nitrate
if combo fails=CABG
B-blockers
unstable angina
at rest, nitro dont work, 20+min,^duration,frequency,severity
NSTEMI
typical CP, ^CKMB/trops,
STEMI ekg changes
ST elevation, T wave changes:inversions, normalizations, hyperacute changes
ST depressions:junctional, downsloping, horizontal
q waves
ACS tx
nstemi cath 24-48hrs, stemi 90 minutes or fibrinolysis
Post MI tx
statin B-blocker ACEi Aspirin 4L clopidogrel prasugrel ticagrelor drug stents=1yr non=3 mo
Inferior MI leads
II,III, AVF
anterior MI leads
V1, V2, V3 ,V4
lateral MI leads
I, AVL, V5, V6
Posterior MI
