lumbar strain 70%, degenerative disk disease 10%, herniated disk 4%, osteoporotic fx 4%, renal/pelvic disease 2%, neoplasm 0.7%
sciatica
pain from low back down leg to calf
low back red flag sx
<20, >50, hx:iv drug, steroid, ca, vImmunity, recent trauma, tb, vWeight, worsens when supine, const sx
back pain physical exam
complete neuro
motor strength, tone, bulk measurements, rapid alternating movements, point to point discrimination, gait
straight leg test
lay down, pain raising leg 15-30 degrees is severe
30-60 moderate
tests L5 and S1
+=herniated disk
when to consider imaging back pain
50+, neuro deficits, CA hx, vWeight, drugs, pain unresponsive to tx, trauma
when to refer back pain
any neuro involvement, recurrent or chronic pain, PT
90% back pain has resolution in
4-6 weeks
