Class
Vasodilator

Mechanism of Action
Smooth muscle relaxant acting on vascular, bronchial, uterine, and intestinal smooth muscle; dilation of arteries and veins in the periphery; reduces preload and afterload; decreases the workload of the heart reducing myocardial oxygen demand.
Nitroglycerin/Nitropaste (Nitrostat)
Indications
Acute angina pectoris, ischemic chest pain, hypertension, CHF, and pulmonary edema.

Contraindicatons
Hypotension, hypovolemia, intracranial bleeding or head injury, previous administration of Viagra, Revatio, Levitra, Cialis, or similar agents within the last 24 hours.

Adverse Reactions
Headache, hypotension, syncope, reflex tachycardia, flushing, nausea, vomiting, diaphoresis, and muscle twitching.

Drug Interactions
Additive effects with other vasodilators; incompatible with other drugs IV.
Nitroglycerin/Nitropaste (Nitrostat) Adult Dosage (Tablets)
Tablets: 0.3 – 0.4 mg SL; may repeat in 5 minutes to a maximum of 3 doses.

Pediatric Dosage
Pediatric: Not recommended

Adult: Paste: 1⁄2 - 1 inch topical

Onset: 1 – 3 minutes

Peak effect: 5 – 10 minutes

Special Considerations
Hypotension is more common in the geriatric population. NTG decomposes if exposed to light or heat. Must be kept in an airtight container.

IV Infusion Dose
IV infusion 10-20 mcg/min, increase by 5-10 mcg q 5 minutes until desired effect.

Transdermal Dose
Transdermal 1-2 cm, 15-30 mg.
