Huntington Dz
Decrease dopamine, pill rolling, cogwheel rigidity, shuffling gait, anticholinergics, Levodopa-carbidopa
Parkinsons
Berry aneurysm, anterior circle of willis, stop smoking, thunderclap HA, meningeal signs + LOC, CT and LP
Subarachnoid hemorrhage (SAH)
Ischemic > Hemorrhagic, Facial paresis, arm drift/weakness, abnml speech, CT scan, TPA <4.15 hours, Angiography
Stroke
Signs and sx of stroke w/o infarct
Transient Ischemic Attack (TIA)
Confusion and amnesia, post-concussion syndrome
Concussion
Ascending weakness/paralysis, absent DTR, campylobacter infection, IVIG or plasma exchange
Guillan barre syndrome
Gradual, progressive, TSH and Vit B12, Cholinesterase inhibitor, Memantine (Namenda)
Dementia
Acute, reversible, fluctuations of consciousness
Delirium
Affects white matter, >2 distinct episodes, women, sensory sx, optic neuritis, oligoclonal bands, glucocoritcoids
Multiple sclerosis
Muscle weakness, acetacholine receptor antibodies, worse at the end of the day, ptosis and diplopia, EMG, Thymectomy <60 or with thymoma
Myasthenia gravis
Postictal confusion, tongue biting, urination, foaming of mouth
Seizure
Petit mal, generalized, children, staring, lip smacking, ethosuximide
Absence
> 5 min, BZD, fosphenytoin or phenytoin, phenobarbital
Status epilepticus
