Adrenal insufficiency and orthostatic hypertension
Mineralocorticoids
RA, severe psoriasis (including arthritis), therapeutic abortion, ectopic pregnancy, crohns disease, dermatomyositis/polymyositis, lupus, ALL, trophoblastic neoplasms, CA (breast, head, neck, lung), T-cell lymphoma, osteosarcoma, and non-hodgkin lymphoma
Methotrexate
RA–mild disease (safe in pregnancy), malaria, and lupus
Hydroxychloroquine
RA–mild dz (sulfasalazine is safe in pregnancy), IBD (mesalamine is considered first line over sulfasalazine [5-ASA derivative] due to fewer adverse reactions secondary to sulfa), and reactive arthritis
Sulfasalazine/Mesalamine
RA–if they cannot take methotrexate or as an add on to methotrexate
Leflunomide
Prevents organ transplant rejection and keeps pts off of corticosteroids in multiple AI dz (RA, crohns, UC, lupus, MS, dermatomyositis/polymyositis, erythema multiforme, pemphigus vulgaris, and psoriasis)
Azathioprine
UC, Crohns dz, RA (not first line), psoriatic arthritis (not first line), and ankylosing spondylitis)
Anti-TNF Drugs (Etanercept, Inflixumab, Adalimumab)
Vasculitis, leukemia, lymphoma, breast CA, nephrotic syndrome, lupus nephritis, and multiple myeloma
Cyclophosphamide
Prevents organ transplant rejection and keeps patients off corticosteroids in multiple autoimmune diseases (Hepatitis, lupus nephritis, myasthenia gravis, and psoriasis)
Mycophenolate
Acute gout prophylaxis (never started during an acute gouty attack)
Allopurinol
Acute gout prophylaxis when the patient cannot take allopurinol
