Gastric cancer

A

Adenocarcinoma - most
Male, chronic Atrophic gastritis and pernicious anaemia
Presentation: Present late with epigastric pain, anorexia, wt loss.
Suspect: onset dyspepsia in over 50yrs with alarm symptoms.
Ass: anaemia
Mets: LN on L side. Ascites, hepatomegaly and bone or brain mets. Or paraneoplastic
Ex: LN, thrombophlebitis migrans or thromboembolic disease, Cachexia, epigastric mass, irregular hepatomegaly and ascites.
Ix: Gastroscopy - ulcer with raised edges or diffusely infiltrative. Barium meal good for diffuse infiltrating carcinoma. FBC, LFT, CXR, USS for liver mets, CT for staging.
Mx : Curative resection not possible in late presentation, 20% survival at 5 years. Palliative chemotherapy
