Achalasia

A

Degeneration of Cells controlling peristalsis and relaxation of LOS
Progressive dysphagia with vomiting several hours after eating.
CXR show fluid line behind heart
Barium swallow dilation with smooth tapering down to LOS
Mx surgery with ballon or myotomy
Botox injections
Medical is last resort - nitrates and ca blocker

Tx of hiatus hernia

A

Symptom relief antacids
Reducing acid to promote healing PPIs and H2 blockers
Increase LOS tone and gastric emptying prokinetic drugs such as domperidone a dopamine antagonist.
