Piedallu’s
purpose: testing for abnormality in torsion of sacrum (meaning checking to see if the sacrum bone is twisted at all that could cause pain)
pt seated
palpate PSIS
have pt slowly bend forward
noting movement of PSIS
positive: if the lower PSIS becomes higher
Gapping (distraction)
purpose: test sprain of anterior SI ligaments
pt supine
cross hands on each hip bone and slowly push apart
positive: one sided glute or leg pain
Yeoman’s
purpose: test for a pathology in anterior SI ligaments
pt prone
bend knee and extend hip up
positive: pain local to SI region
Prone Gapping (Hibbs)
purpose: test integrity of posterior SI ligaments
(hips and knees must have full ROM and no pathology)
pt prone
examiner stabilizes pelvis
pt knee flexed to 90 & hip is rotated medially as far as possible and palpating SI joint on same side
positive: pain in SI region
Prone Knee Bending (Nachlas)
purpose: test for tight rec fem, nerve root lesion, SI lesion, hypomobile SI joint
pt prone
flex knee to buttocks
positive: pain felt in the front of the leg indicating a tight quad muscle (rec fem)
Squish Test
purpose: test for stability of posterior SI ligaments
pt supine
therapist places hands on hip bones and pushes in and down at 45 degree angle
positive: pain around pelvic region
sign of buttock
purpose: test for pathology behind hip joint like bursitis, absess or tumour
pt supine
test range first with SLR
flex knee and bring towards their head
positive: if hip cant flex and the knee doesnt come any closer towards the head
Flamingo
purpose: test for SI joint lesion or lesion in pubic symphysis
pt standing holding onto chair
slightly lifts one leg off ground
if no pain have them hop up and down a couple times
positive: pain in pelvic region (pubic symphysis or SI) wherever pain is felt is where lesion is
Gillets
purpose: test for hypomobile SI joint
pt standing
therpaist sitting and palpate PSIS with one thumb and sacrum w other
have them lift leg of PSIS being palpated
positive: if PSIS doesnt move or it moves upward
Sacroiliac rocking
knee to shoulder test
sacrotuberous stress test
purpose: assess sacrotuberous ligament
ask if they have any hip or knee pathologies and have full ROM
pt supine
flex their knee and bring up towards opposite shoulder and move around in a few directions
positive: pain in SI region
Gaenslens
purpose: test same side SI joint lesion, hip pathology, L4 n. root lesion
pt lies on side and holds bottom leg to chest
upper leg is extended up and back by therapist
positive: P on side being tested
Trendelenburg
purpose: asses for instability/weakness in glute med
pt standing, holding onto chair for support
lifts one knee up to chest
examiner watching to see pelvis of lifted leg side collapses down
positive: if pelvis on lifted knee side falls - considered positive and indicates glute med weakness
Kemps
purpose: test for facet joint irritation
pt standing, ask them to rotate and slide hand down midway on the back of thigh
examiner stabilizes pelvis and applies downward pressure on their shoulder
positive - pain in low back or down leg of side being tested
