May 21st Flashcards
Renal angiomyolipoma is associated with what genetic disease?
Tuberous sclerosis
Child with abdominal pain and purpuric rash on lower extremities and arthralgia
Think Henoch-Schonlein Purpura; IgA mediated leukocytoclastic vasculitis
What laboratory findings are seen in Henoch-Schonlein Purpura?
Normal plts, normal coags, normal to elevated Cr, hematuria + proteinuria
What is the treatment for Henoch Schonlein purpura?
Supportive, NSAIDs, if severe steroids
What is lung compliance?
Ability of lungs to expand
What pathophysiology is seen in ARDS?
V/Q mismatch; lung compliance (ability to expand) is decreased (stiff lungs) due to both loss of surfactant and increased elastic recoil of edematous lungs; pulm art pressure increased (PHTN) due to hypoxic vasoconstriction
What is attributable risk percentage?
Represents the excess risk in a population that can be explained by exposure to a particular risk factor
What is the calculation for attributable risk percentage using RR?
ARP = (RR-1)/RR
what respiratory changes are seen in pregnancy?
Increased ventilation due to increased TV and increased minute ventilation; results in chronic respiratory alkalosis with increased PaO2 (100-110); all due to progesterone
In a case control study what is the main measure of association?
Exposure odds ratio
What is the proper protocol for post-exposure prophylaxis to HIV?
Immediate serology; serology at 6 weeks, 3 months, and 6 months; and triple drug therapy (tenofovir-emtracitabine with raltegravir preferred; ie 2 NRTIs + integrase inhibitor); PEP x 4 weeks
What is the adverse effect of hydroxychloroquine?
Retinal toxicity; baseline ophthalmologic exam with annual reassessment beginning after 5 years
What are the adverse effects of MTX?
Liver tox, pulm fibrosis, stomatitis, bone marrow suppression
What is a normal Rinne test? An abnormal Rinne test means what?
Normal: AC > BC ; abnormal BC > AC (hear it more on mastoid vs EAC), abnormal means conductive hearing loss
In conductive hearing loss, the Weber test results in what?
Lateralization to affected ear
