Lower Limbs Flashcards
Non-invasive investigation of lower limb ischaemia?
Measurement of ABPI
Duplex ultrasound scanning
Invasive investigations for lower limb ischaemia?
Magnetic resonance angiography
CT angiography
Catheter angiography
How do you calculate ABPI?
Ankle pressure / brachial pressure
What treatment and advice can slow the progression of lower limb ischaemia?
SMOKING Lipid lowering Anitplatelets Hypertension Diabetes Life style issues
What treatment can be used to improve the symptoms of claudication?
Exercise training
Drugs
Angioplasty/stenting
Surgery
Symptoms of critical limb ischaemia
Pain at rest Toes and forefoot Requires strong analgesia Worse at night Helped by putting leg in a dependent position Helped by getting up and walking about
2 main risk factors for amputation
Smoking
Diabetes
What is intermittent claudication?
When insufficient blood reaches exercising muscle
What is the typical presentation of intermittent claudication?
Patient is pain free at rest but after variable periods of exercise develops ischaemic pain
What are the 2 parallel venous systems in the legs?
Deep system (tibials, popliteal, femoral) Superficial system (sap heinous and perforators)
Risk factors for varicose veins
Age
Pregnancy
Obesity
Family History
Investigation for varicose veins
Duplex scanning
Complications of varicose veins
Bleeding & bruising Superficial thrombophlebitis Chronic venous insufficiency Ulceration Haemosiderin deposits Lipodermatosclerosis
Management of varicose veins
Compression (stockings/bandage) Endovenous (duplex guided) - foam sclerotherapy - ablation Surgical
When is graduated compression contraindicated in varicose veins?
Patients with low ABPI
