What is LOTO?
Lock off tag out is a safe practise of work, it requires a list of precautions before isolating a system, machinery or equipment. LOTO is in place to prevent harm to the crew or damage to the ship, it must be followed in order to ensure the safety of everyone onboard. It also allows seniors rates to track the work of their branch, detailing information on jobs such as dates and times and how and why the job is being conducted.
How is a LOTO conducted?
Permission is obtained from senior rate
Isolation points must be located and personal affected must be informed
Obtain serial number and fill out index sheet and LOTO labels, once signed return the index sheet to section 5 of LOTO log and place labels A on the isolation point, B on the board and C on the key to the isolation point
Once the job is complete fill out the rest of the index sheet, get signed off and return labels to senior rate for description. Return index sheet to part 6 of LOTO log
