Long Bone Fractures 2 Flashcards
Generally seen in farm animals d/t dystocia or in group housed juvenile cattle
femoral physeal fracture
How do you treat a femoral physeal fracture in a cow?
Stall rest and ORIF w/pins/screws
How do you insert IM pins for femoral head fixation?
Through femoral neck from caudolateral femur
How do you treat a femoral physeal fracture in a foal?
Dynamic hip screw system
Is FHO successful in large animals?
No
Severe lameness w/crepitus and pain of the stifle
Tibial fracture
How do you treat a SH type 2 tibial fracture in a farm animal?
Thomas Shroeder splint for 6wk
How do you treat a SH type 2 tibial fracture in a foal?
Plate and screw fixation w/tension band and suction drain
What type of screws do you use for fixation of a SH type 2 tibial fracture in a foal?
Cancellous
When can you proceed w/surgical repair of a mid-diaphyseal humeral fracture in a cow?
After 8wk stall rest
Potential complications of this treatment option include non/mal union, breakdown or laminitis of opposite limb
Stall confinement
How do you treat a humeral fracture in foals?
ORIF w/plates
What nerve is at risk when doing an ORIF on the humerus?
Radial nere
Are interlocking nails adequate treatment for adult animals?
No
Why would you not want to splint/bandage a humeral fracture?
Bandage/cast can’t actually reach the fracture so it just adds weight to the limb
When trailering a broken horse for transport, which direction should they face?
Broken limb facing out
You can’t adjust placement of this kind of cast after application and restricts your access to the soft tissue
Transfixation pin cast
Systemic abx, general anesthesia, and dorsal recumbency are needed for placing this type of cats
Transfixation pic cast
How many pins per fragment is required for stabilization of a short oblique fracture?
2
Used for comminuted phalangeal fractures
Transfixation pin casting
How should pins of a transfixation pin cast diverge and why?
30* divergence in frontal plane to decrease risk of fracture between pins
Which ESF has pins placed laterally to medially?
Type 2 external fixator
Which ESF has cranially to caudally directed half-pins?
Type 3 external fixator
What type of ESF do you use for tibial fractures?
Type 3
How much space do you need between the skin and a cast (padding)?
1/4 inch minimum
When do you remove ESF pins?
6-8wk
How do you repair an antebrachium fracture?
Repair radius and ulna will follow
What is plate luting?
PMMA application
Bone cement
PMMA
How does PMMA increase stability of a plate?
Creates 100% plate to bone contact
