Goal – Overall purpose

Objectives – Desired outcomes
Activities/Outputs – What’s done
Resources/Inputs – What’s needed
Optional Components
Context – Background situation
Indicators – Track outcomes & progress
Assumptions – Beliefs about program success
External Factors – Outside influences (e.g., policy, funding)

Reduce falls & injury in older adults
