rats
What is the pathogenic leptospirosis species?
L. interrogans
What is the saprohytic lepto species?
L. biflexia
most important manifestations of leptospirosis
vasculitis
most impt known pathogenic properties of leptospires
cell surfaces and cellular toxicity
The major surface lipoprotein seen among pathogenic serovars
LipL32
First genetic susceptibility factor identified
HLA DQ6
WHO Criteria for Definitive Leptospirosis
a. Positive leptospiral culture of the blood and/or urine
b. High positive single lepto Microagglutination Test (MAT) titer or 1: 1600 (Phils. has higher cutoff)
c. Seroconversion from initial MAT negative to a positive antibody titer
d. Fourfold rise in antibody titers by MAT from acute to convalescent
Systemic reaction resembling bacterial sepsis
Jarisch Herxheimer Reactions
What level of exposure? single history of wading in flood or contaminated water with wounds, cuts, or open lesions of the skin OR accidental ingestion of contaminated water
moderate-risk
What level of exposure?single history of wading in flood or contaminated water without wounds, cuts or open lesions of the skin
low-risk
first sign of pulmonary leptospirosis activity?
tachypnea
doxycycline side effect
sunlight sensitivity, less effective OCP
management of AKI in leptospirosis
methylprednisolone
treatment for mild lepto
doxycycline
