What are the pharmacological actions of Ketamine?

A

Production of a dissociative state characterised by:

  1. A trance-like state with eyes open but not responsive
  2. Nystagmus
  3. Profound analgesia
  4. Normal pharyngeal and laryngeal reflexes
  5. Normal or slightly enhanced skeletal muscle tone
  6. Occasionally a transient and minimal resp. depression

A
  1. Any condition where a significant elevation of BP would be hazardous, eg:
    - HTN
    - CVA
    - Recent AMI
    - CCF
  2. If being administered for analgesia, inject slowly over 1 min to reduce risk of respiratory depression and hypertension

List the side effects of Ketamine.

A

CARDIOVASCULAR
1. Increase in BP and HR

CNS

  1. Respiratory depression or apnoea
  2. Emergence rxns (nightmares, restlessness, vivid dreams, confusion, hallucinations, irrational behaviour)
  3. Enhanced skeletal tone
  4. Nausea and vomiting

OCULAR
6. Diplopia and nystagmus with slight increase in intraocular pressure

OTHER

  1. Lacrimation
  2. Local pain at injection site
  3. Hypersalivation

A
  1. Emergence rxns associated with Ketamine for analgesia in the adult pt can be managed with Midazolam.
  2. Consultation w/ RCH is required for Midaz admin in paediatrics.
  3. Hypersalivation can be managed with suction or Atropine if severe.
