What is the presentation of Heparin?
A
5000 units in 5mL ampoule.

What is the pharmacology and action of Heparin?
A
Anticoagulant.
Inactivates clotting factors IIa (thrombin) and Xa by binding to antithrombin III.

How is Heparin metabolised and excreted?
A
Metabolised by the liver, excreted by the kidneys.

List the PEI for Heparin.
A
- Acute STEMI

List the contraindications for Heparin.
A
KARBOSH
- Known allergy or hypersensitivity
- Active bleeding (exc. menses)
- Oral anticoagulants
- Bleeding disorders
- Hx of heparin-induced thrombocytopaenia (HIT)
- Severe hepatic impairment/disease inc. oesophageal varices.
- Recent trauma or surgery <3wks

A
- Renal impairment

List the side effects of Heparin.
A
- Bleeding
- Bruising and pain at the injection site
- Hyperkalaemia
- Thrombocytopaenia (mild to severe)

List the special notes for Heparin.
A
- Do not inject IM due to risk of haematoma.
2. If time to PCI >1hr, pt will require rpt doses due to half-life of 60mins
