Immunosuppressant Flashcards
Corticosteroids
Prevent IL1 and 6 from macrophages, prevent T cell activation therefore depress immune response
Azathioprine
Suppress inflammatory cystokines
Use of azathioprine
SLE and vasculitis
Testing before azathioprine
TPMT highly polymorphic- if low there is increased risk of myelosuppression
Ciclosporin and tacrolimus
Calcineurin inhibitors- prevent production of IL2
Mycophenolate mofetil
Inhibits inosine monophosphate dehydrogenase and therefore inhibits B and T cell proliferation. Spares other cells
Use of Mycophenolate
Lupus
S/E Azathioprine
Myelosuppression, Malignancy, hepatitis
Cyclophosphamide
Alkalating agent, cross liks DNA so no replication. Decreased B and T cell activity.
Use of Cyclophosphamide
Lupus. solid cancers, lymphoma
Methotrexate
FOR CANCER- Competes and reversibly inhibits dihydrofolate reductase. inhibits DNA, RNA and protein synthesis during S phase.
Use of methotrexate
RA
Special about methotrexate
Weekly dosing, NSAID interaction
Sulfasalazine
Relieve pain and stiffness and fights infection by T cell inhibition and decrease chemotaxis and degranulation
S/E Cyclophosphomide
Hemorrhagic cystitis in bladder - MESNA or hydration needed
