ILD Flashcards
What is interstitium
-it’s the area between the capillary endothelium and alveolar epithelium
What are the types of interstitium
-axial
-peripheral
-centrilobular
What is axial interstitium
-it’s the interstitium that covers the bronchus to respiratory bronchiole
What is peripheral interstitium
-it’s the interstitium that covers the pulmonary veins, lymphatics and septae
What is cetrilobular interstitium
-it’s an interstitium that covers from capillary endothelium to the alveolar epithelium
What are the types of ILD
-known
-unknown
-granuloma
What are the known ILD
-drugs (Bleomycin, Amiodarone)
-exposure (Pneumonoconiosis and HP)
What is the unknown ILD
-Idiopathic Pulmonary Fibrosis
What is the granuloma ILD
-sarcoidosis
What is the risk age of IPF
-middle age (40-70)
What are symptoms of IPF
-dry cough
-SOB
What are the signs of ILP
-Velcro crackles
-digital clubbing
What is the X-ray finding of ILP
ABBBB
-asymetrical
-abnormal >90%
-bilateral
-basal
-peripheral
What is the X-ray pattern of ILP
-reticular
-reticulonodular
What is the CT finding of early ILP
-patchy, peripheral, reticular subpleural and basal
-ground glass
What is the CT finding of late ILP
-honey comb appearance
What is the function of BAL and TBB in ILP
-to rule out alternate diagnoses
What are the major criteria of ILP
-exclusion of known ILD
-abnormal lung function
-hypoxia
-CT: bibasal reticular
-BAL and TBB shows no alternate diagnoses
What are the minor criteria of ILP
-age>50
-dyspnea
-ilness>3 months
What are the ttt for ILP
-steroids
-cytotoxic drugs (azathioprim, cyclophosphamide)
-anti-fibrotic (calchicine, prifenidone)
What are the indications of good prognosis in ILP
-age<50
-female
-symptoms<1 year
-CT; ground glass
What is the contraindication of lung transplantation
-age>60
-mental unstable
-extrapulmonary disorder
