What is the MOA for Ibuprofen?
Ibuprofen is a non steroidal anti inflammatory used for treating pain
Ibuprofen inhibits the prostaglandin synthetase reducing prostaglandin production and reduces inflammation, pain and fever
What are the indications for Ibuprofen?
mild to moderate pain, usually in combination with paracetamol) esp soft tissue, musculoskeletal pain or headache
May be administered in combination with other drugs for severe pain, esp during long transport times
What are the contraindications for Ibuprofen?
Known severe allergy
Third trimester of pregnancy
What are the cautions for ibuprofen?
- taken within last 4 hours
- abdominal pain esp where risk of significant intra-abdominal illness exists
- Age>75 yo esp in presence of illness, infection or dehydration (risk of worsening renal impairment)
- Dehydration or shock (risk of worsening renal impairment)
- Known renal impairment
- Known bleeding disorder
- Clinically significant bleeding (Ibu will increase the risk of bleeding)
- Known worsening of bronchospasm with NSAIDS in patients with asthma or CORD
- taking warfarin (ibu increases the activity of warfarin), this can be significant when bleeding is involved
- pregnancy
Should Ibuprofen be given during pregnancy?
Generally should be withheld because of availablity of other forms of pain relief and danger to fetus and delivery
What doseage of Ibuprofen should be used?
800mg for adults >80kg
600mg for adults 80kg or less
see paediatric drug dose tables for a child, tablets may need to be crushed and put in jam
What is prostaglandin and what is its function?
Prostaglandins are lipids with hormone like effects. They have a range of functions including:
- sensitizing nerve cells to pain stimuli
- induce fever
- regulate inflammation
- induce labour
- stimulate mating behaviour in goldfish
What are common adverse effects of ibuprofen?
- Renal impairment
- Increased bleeding
- indigestion and GI ulceration/bleeding usually only associated with chronic use
