Hydrocortisone Flashcards
what are the pharmacokinetics for hydrocortisone
administration: parental
distribution: plasma protein
metabolism: liver (and my most tissues)
excretion: urine
what are the pharmacodynamics for hydrocortisone
glucocorticosteroid: inhibits inflammatory effects
also has an antihistamine effect by reducing histamine release
suppresses cell-mediated immunity and suppresses humoral immunity
what are the 3 ways hydrocortisone can be presented on the road
solution for injection: hydrocortisone sodium phosphate 100mg/1ml solution for injection ampoules
powder for solution for injection: hydrocortisone sodium succinate 100mg powder for reconstitution with up to 2ml of water
ampoule of 100mg hydrocortisone sodium succinate for reconstitution with up to 2ml of water
what are the indications for hydrocortisone
severe or life threatening asthma
acute exacerbation of COPD
adrenal crisis, time critical emergency with associated mortality
prevention of adrenal crisis for pt who suffer trauma or acute illness and are long-term steroid therapy either:
.as replacement therapy for adrenal insufficiency
. long term therapy at doses of 5+mg prednisolone (like immune suppression)
at are the other 3 types of pt hydrocortisone should be given to
established adrenal crisis (ensure parenteral hydrocortisone is given prior to transportation)
suspected adrenal insufficiency or on long term steroid therapy who are unwell, to prevent from having adrenal crisis
pregnant pt with known additions disease who are in established labour WITH regular painful contractions
should you give hydrocortisone in a post rosc pt who had arrested due to adrenal crisis
yes
what does hydrocortisone do to a pt
its a glucocorticoid drug that restores bp, blood sugars, cardiac synchronicity and volume
high levels= important to survive shock
therapeutic actions include suppression of inflammation an immune response
what are the contra indications for hydrocortisone
known allergy
when pt is in adrenal crisis, preferable to give whatever preparation possible
what are the cautions for hydrocortisone
single shot
avoid IM if pt is likely to require thrombolysis
what are the side effects for hydrocortisone
contains high amounts of phosphate preservative and can cause stinging/burning
if you are unsure if pt has already had steroids should you still give hydrocortisone. is there a toxic dose for hydrocortisone
yes
there is no toxicity level to hydrocortisone
what is the route of administration for hydrocortisone
IV OR IM (in upper arm or thigh)
should u administer hydrocortisone quick or slow for IV
slowly over 2 minutes to help stop burning sensation
what is the initial dosage for hydrocortisone
100 milligram (mg)
