Amiodarone Hydrochloride (CARDIAC DRUG) Flashcards
What is the pharmacodynamics
What are the pharmacokinetics for amiodarone
administration: parental
distribution: plasma protein, widespread, especially liver and adipose tissue
metabolized: liver and bile
Excreted: faeces
what are the pharmacodynamics for amiodarone
Anti-arrhythmic:- it lengthens cardiac action potential and therefore effective refractory period
prolongs QT interval on ECG
Blocks sodium and potassium channels in cardiac muscle
acts to stabilize and reduce electrical irritability of cardiac muscles
what are the indications for amiodarone
a shockable rhythm following 3rd and 5th unsuccessful defibrillation attempts
what are the contraindications of amiodarone(UPDATED)
if the pt’s core temp is below 30 degrees
what are the cautions for amiodarone
none
what are some adverse effects that amiodarone can cause
bradycardia
vasodilation causing hypotension
bronchospasm
arrhythmias (torsades de pointes )
what is the adult dosage for amiodarone and when should it be administered
300 milligrams (mg) on 3rd shock
150 milligrams (mg) on 5th shock
What is the max dose of amiodarone for adults
450 milligrams (mg)
How is amiodarone administered
must be well flushed (20ml of sodium chloride) given in rapid bolus.
Must be given in large vein because it can cause burns
what is the route of administration for amiodarone
IV OR IO
how is amiodarone presented on the road
pre-filled syringe containing 300 milligrams (mg) of amiodarone in 10ml
AND
ampoule 150miligram(mg) in 3ml
what are the 2 rhythms that amiodarone should be given in cardiac arrest? is this shockable or unshockable?
shockable rhythms: Pulseless VT OR VF
After which shock should amiodarone be administered
if shockable rhythm- 3rd and 5th only
