What can happen with hidradenitis suppurative?
abscess formation, sinus tracts
What are the scars from this?
hypertrophic scars, keloid scars
What is the pathogenesis of this?
keratin plugging of apocrine duct and hair follicle –> dilation of duct –> inflammation of apocrine gland –> bacterial overgrowth
What bacteria can overgrow in this?
s. aureus, streptococci, e. coli, proteus mirabilis, pseudomonas aeruginosa
What is the treatment drug wise?
PO antibiotics Tetracycline Doxycycline Erythromycin minocycline Accutane - early dz
What are some other treatments of hidradenitis suppurative?
intralesional injections with triamcinalone
surgical - marsupializtion, surgical excision, grafting
What are comedones that appear at scar site?
tombstone comedone
How can you differentiate this from a furuncle?
recurs at same spot, furuncle does not
