First line of Tx for gout
NSAIDS
Indomethacin/Indocin
NSAID for gout and other inflammatory conditions.
Normal NSAID sides
can also exacerbate HTN or CHF
Do not use with history of nasal or sinus polyps
Cortisone injection
Steroid for short term pain relief and decreased swelling
Given by intraarticular injection (also PI, IM, IV, spray, or top)
Sides are typical steroid sides like hyperglycemia and dm,
osteoporosis, depression, etc
Colchicine
mitotic inhibitor for acute gout and gout prophylaxis
binds to tubulin and inhibits mitosis. also inhibits neutrophil motility
sides:
PERIPHERAL NEUROPATHY
Gi stuff, neutropenia and hair loss
overdose> vomiting, renal failure, hypovolemic shock
Allopurinol/Zyloprim
purine analog for hyperuricemia and gout
inhibits xanthine oxidase
Can cause n&v, SJS, and rash
Hypersensitivity causes the rash with a fever, hepatitis, and decreased renal fxn
Probenecid/Probalan
Uricouric for hyperuricemia and gout
decreases reabsorption of uric acid so it is excreted in the urine
can cause uric acid renal stones
pt with G6P deficiency> anemia, leukopenia
gout may reappear after stopping medication
do not use with salicylates
