- Fluid
- pRBC if HCT under 30 in old/CAD
- FFP if PT/INR elevated
- Platelets if below 50K and active bleed
- Octreotide if variceal
- EGD

When to do nuclear bleeding scan?
Angiography?
Capsule endoscopy?
CT/MRI?

Endoscopy shows nothing in massive acute bleed
Need to find vessel for surgery in massive unresponsive hemorrhage
Nothing seen with upper and lower endoscopy
Not useful
