Age
Appears to be stated age
Sex
Sexual development appropriate for gender and age
LOC
Alert and oriented, listening and responding to my questions appropriately
Skin colour
Even colortone, intact skin with no obvious lesions
Facial features
Symmetrical with movement
Distress
No sign of acute distress
Stature
Height within normal range for age and genetic heritage
Nutrition
Weight within normal range for height and body build, even distribution of body fat
Symmetry
Body parts equal bilaterally and in relative proportion to each other
Posture
Standing comfortable erect as appropriate for age; normal plumbline through anterior ear, shoulder, hip, patella and ankle.
Position
Sitting comfortably on the bed, arms relaxed at sides, head turn to me
Body build, contour
Normal proportions: a) arm span fingertip to fingertip equals height;
B) body length from Crown to pubis approximately equal to length from Pubis to sole
No obvious physical deformities
Gait
Base width equal to shoulder width.
Accurate foot placement; smooth, even, and well-balanced walk; and symmetrical arm swing when patient came in.
ROM
Full mobility in each joint, and deliberate, accurate, smooth, and coordinated movement
Facial expression
Maintaining eye contact with me, expressions are appropriate for the situation.
(Note expressions both while the face is at rest and while the patient is talking. E.g., thoughtful, serious, or smiling)
Mood and affect
Comfortable and cooperative with me and interacting pleasantly
Speech
Clear and understandable articulation.

Dress
Clothing appropriate for the climate, looks clean and fits the body and appropriate for the patients culture in age group
OR
patient dressed in Bow Valley college uniform, appropriate for situation looks clean and fits the body
Personal hygiene
Appearance: clean and groomed properly for patient’s age.
Hair groomed, brushed. Makeup is appropriate for age
