A
  • Weak Tibialis Anterior
  • weak peroneus brevis
  • Strong Peroneus Longus overpowers tibilalis ant-> dropped 1st ray
  • Strong Tibialis posterior overpowers peroneus brevis- hindfoot varus and forefoot adduction, forfoot pronation

  • In stroke victims

TX

  • USe AFO and physio for at laest 6 months to see if any return of neuro

Non op

  • Phenol/ Botox injections

Surgery

  • Split anterior tibialis tendon transfer
  • FHL transfer to dorsum of foot and release FDL and Brevis at base of each toe
  • Lengthen gastronemius/ achilles
