How does the Progestogen-Only Implant function? (A3).
It slowly releases Progestogen into the systemic circulation which :-
- Inhibits Ovulation.
- Thickens Cervical Mucus.
- Alters Endometrium and Makes it Less Accepting of Implantation.
What is the Implant?
A small (4cm) flexible plastic rod placed in the upper arm just between the skin and the subcutaneous fat.
Efficacy of Progestogen-Only Implant (2).
- Most Effective Form.
2. > 99% with Typical and Perfect Use.
How must the POI be inserted? (3).
- Lidocaine (LA).
- 1/3 of the way up the upper arm on the medial side.
- Specially Designed Device used to insert it horizontally.
How must the POI be removed? (4).
- Lidocaine (LA).
- Small incision at one end on skin.
- Pressure on other end or using forceps.
- Immediately use contraception.
How do you know if an implant has been successfully inserted? (2)
- Press on one end to make the other end pop upwards.
2. Palpable immediately after insertion.
What is the only UKMEC4 Criterion with POI use?
Active breast cancer.
What is the implant used in the UK?
Nexplanon - contains 68mg of Etonogestrol.
How long does the implant last for?
3 years before replacing.
Protection Offered by Implant (2).
- Start on Day 1-5 for immediate protection.
2. If no, 7 days of extra contraception is required e.g. condoms.
Adverse Effects of Implant (3).
- Acne.
- Problematic Bleeding.
- Impalpable/Deeply Implanted.
What happens if it becomes impalpable? (3).
- Extra contraception is required until located.
- US/X-Ray required to locate it after being referred to specialist removal centre.
- Contains BaSO4 (radio-opaque).
Indications of Implant (2).
- Between 18 and 40 years.
2. Improve Dysmenorrhoea.
Interactions of Implant.
Anti-Epileptics and Rifampicin can reduce efficacy.
