Why may emergency contraception be required?
After episodes of UPSI (Unprotected Sexual Intercourse) and if methods are not fully effective e.g. damaged condoms, multiple missed pills.
What are the 3 main options for emergency contraception?
- Levonorgestrel (within 72 hours of UPSI) - Levonelle.
- Ulipristal (within 120 hours of UPSI) - EllaOne.
- Copper Coil (within 5 days of UPSI or 5 days of the estimated date of ovulation).
* 1/2 are also known as the Emergency/Morning-After Pill.
What is the fate of the Copper Coil, after use as emergency contraception?
It should be kept in until at least the next period, when it can be removed or be used as long-term contraception.
What is the dosage of Levonorgestrel used?
Single dose of 1.5mg (or 3mg in women above 70kg or BMI above 26).
What happens to COCP/POP use after using Levonorgestrel as Emergency Contraception?
It can be started immediately after taking Levonorgestrel but extra contraception is required for the first 7 days with the COCP and 2 days with the POP.
Can you breastfeed after taking Levonorgestrel?
Yes, but it is recommended to be avoided for 8 hours after taking the dose to reduce exposure to the infant.
What is the mechanism of action of Ulipristal?
SERM (Selective Progesterone Receptor Modulator) - it works by delaying ovulation and is more effective than Levonorgestrel.
What happens to COCP/POP use after using Ulipristal as Emergency Contraception?
The lady must wait 5 days and as usual take extra contraception for the first 7 days (with the COCP) and 2 days (with the POP).
What is the dosage of Ulipristal?
Single dose of 30mg.
Can you breastfeed with Ulipristal?
Avoid for 1 week after taking Ulipristal (milk should be expressed, but discarded).
Give 1 contraindication to Ulipristal.
Severe asthma.
