Postpartum Assessment?
Vital Signs: BP, Temp, Pulse, Resp
Lung sounds: clear or adventitious lung sounds (crackles?)
BUBBLE - HE
Postpartum bleeding, objective, subjective and fundal massage?
- Subjective measurement:
scant, light, moderate, heavy - Objective measurement:
Hct & Hgb
weighing clots and other items
that may be soaked with blood - Gentle fundal massage
maintain/restore uterine tone
How do you palpate the fundus?
Place upper hand above the fundus
Dip lower hand right above the
symphysis pubis
What is important to know for kegel exercises?
Teach your client how to do Kegel Exercises:
-Make sure the bladder is empty
-Tighten the pelvic floor muscles. Hold tight and count 3-5 seconds
-Tighten like you are trying to hold it and you need to urinate badly, You should feel your muscles tighten as you do this
-Relax the muscles and count 3 to 5 seconds
-Repeat 10 times, 3 times a day (Morning, afternoon and at night)
strengthens the muscle tone from stretched and/or torn pelvic tissue and reduces urinary incontinence
What is the health promotion for Rh immune globulin: RhoGam?
- given to Rh neg mothers at 28 weeks gestation
- and again within 72 hours after birth if Rh+ baby
- usual dose 300mcg IM
-made from human plasma (Jehovah’s witness?)
What is the health promotion for Rubella Vaccine (MMR: measles, mumps, rubella vaccine)?
- SQ prior to discharge
- prevent future rubella infection during pregnancy
- live vaccine – use birth control for at least 4 weeks
- may continue to breastfeed
What is the health promotion for Varicella vaccine (chickenpox vaccine)?
- SQ prior to discharge
- live vaccine
- 2nd dose in 4-8 weeks
What is the health promotion for the Tdap vaccine (tetanus-diphtheria-acellular pertussis)?
- IM
- decrease infant exposure to pertussis (whooping cough)
- recommended by CDC in every pregnancy
