Equine wound management Flashcards
1
Q
What 4 stages of wound healing?
A
- Inflammatory phase
- Debridement phase
- Proliferative/fibroplastic phase
- Maturation phase
2
Q
Describe horse problem 1: synovial sepsis
A
- Synovial cavities with thin soft-tissue coverage
- Joints/ tendon sheaths/bursar
- If untreated: sepsis - severe pain - articular cartilage destruction - non-treatable, infective, osteo-arthritis
- Required rapid surgical lavage and intensive AB tx
3
Q
Diagnosis of Synovial sepsis?
A
- Sudden onset, severe lameness
- Turbid synovial fluid
- Synovial WBC >20x109 /l
- Protein >20g/l
- Egress of injected sterile fluid
4
Q
Horse problem 2: proud flesh formation?
A
- Exuberant granulation tissue
- Deforms wound
- Grows beyond wound edges
- Can look like sarcoid
5
Q
What is horse problem n°3?
A
No wound contraction below carpus/ tarsus
6
Q
Describe wound problem n°4
A
- Sequestrum = piece of dead bone
- Involucrum = hole in parent bone
- Periosteal damage often results in sequestrum formation
- Acts a nidus” for infection
- Wound can heal over the tip of sequesturm - recurrent wound breakdown and discharge
7
Q
What does sequestrum formation look like?
A
8
Q
Horse problems 5?
A
MOVEMENT
- Impedes wound healing
- Prevents wound edges from remaining apposed
- Encourages spread of infection
9
Q
What to achieve with wound dressings?
A
- Protect wound
- Haemostasis
- Remove exudate or necrotic tissue
- Encourage granulaiton tissue
- Encourage epithelialisation
10
Q
How do we deal with infeciton?
A
- Lavage
- Drainage
- Surgical debridement
