Preventative Measures
Kegel exercises
Perineal massage
Natural pushing during labor, and avoiding the lithotomy position or
pulling back on legs
Side-lying position for pushing
• result in less postpartal discomfort
• added laceration may extend into the anal sphincter
• limited enlargement of the vaginal opening because perineal length is
limited
MEDIAN
• is begun in the midline but directed laterally away from the rectum
• more enlargement of the vaginal opening
• little risk that it will extend into the anus
• more blood loss
• increased postpartum pain
