Purpose of Amniotomy
As a method of induction of labor during the 1st stage
2.) Augment labor (Prostaglandin helps stimulating labor)
3.) To allow access to the fetus to apply an internal fetal heart monitoring electrode to fetal scalp
An artificial rupture of the amniotic membranes (AROM)
Most common operative procedure
Instrument is inserted through the cervix at least 3cm of cervical dilatation
Safest, easiest and surest way to induce labor
Amniotomy (Uses amniotome or amniohook)
Risks
1.) Prolapse of the umbilical cord
2.) Infection
3.) Abruptio Placenta
Procedure
1.) While performing a vaginal exam, MD, RN introduces an amniohook into the vagina
2.) A small tear is made in the amniotic membrane
3.) Following rupture of the membrane, amniotic fluid is allowed to escape
____ if the presenting part is high in the pelvis or the presenting part is not cephalic
Deferred
When is the best time to rupture amniotic fluid membranes?
During relaxation and not contraction.
Purpose 2
To obtain fetal scalp blood sample for acid base determination
To assess the color and composition of amniotic fluid
Management
1.) Patient is in semi reclining position and draped to provide privacy
2.) FHR monitored before and immediately after and they are compared
3.) Amniotic fluid is inspected for amount color, odor and presence of meconium or blood
Management 2
1.) Clean and dries perineal area and change under pads
2.) Temp is monitored every 2 hours
3.) If meconium is present, inform mother to expect special intervention at birth
4.) After birth if meconium is present, vocal cord of baby is visualized and deep suctioning is done
