Endoscopic Instruments Flashcards
Laparoscopic Access & Dissection Devices
Accessory
- Dissecting balloon uses a distention balloon to separate tissues forming a discrete cavity for laparoscopic inguinal hernia repairs
Camera
Viewing
- Pass the plug end of the camera off the sterile field to plug into a camera box
- Sterilized by soaking in Cidex, unless clearly marked βautoclavableβ
Light Cord
Accessory
- Tight bundle of fiber optic strands of glass that transmits light into enclosed space
- Reduction of light conduction can result from broken fiber optics from being coiled too tightly
Laparoscope
Viewing
- Endoscope placed through a trocar sleeve into the abd to view organs
- The outer edge is surrounded by light fibers that pass through the light cord and illuminate the abdomen.
Variable size, diameter and lens degree
Veress Needle
Supply
- Introduces CO2 gas into the abd to distend abdominal cavity, allows visibility during laparoscopic procedures
- Inserted into initial incision site then attached to insufflation tubing
regular or long
Trocar w/ Sleeve
Supply/Retracting/Viewing/Cutting
- Trocar : used to dissect through abdominal tissue to access to abd cavity
- Sleeve : provides leak free seal to protect the skin & tissue layers from damage
variable size (5, 10, & 12 mm)
different styles
Insuflation Tubing
Supply
- Tubing attached to CO2 gas pump at 1 end & other is attached to veries needle
- Tubing has an inline filter to reduce contaminants from entering surgical site
Trocar Anchoring System
Supply
- Used in open technique when a blunt trocar w/ and anchor is inserted w/o prior use of peritoneum
- Sutures are placed on both sides of the sleeve & wrapped around the anchor
SILS Port
Supply
- Flexible laparoscopic port that can accommodate up to 3 instruments through single incision `
Reducer Cap
Supply
- Device clips on top of a 12/10 mm trocar sleeve allowing 5 mm instruments to be used in larger ports w/o loosing pressure
Micro Scissors
Cutting/Dissecting
- Used to make very small cuts in fine structures such as cystic duct
Hook Scissors
Cutting/Dissecting
- Hook tip helps to keep slippery structures w/in the jaws of the scissors while cut is made
Endoshear Scissors
Cutting/Dissecting
- Frequently used very sharp dissecting scissor.
- Used similarly as a metz scissor
variable size (5/10mm)
Endo Grasper
Grasping/Holding
- Traumatic: has a wide jaw to grasp & manipulate larger amounts of tissue
- comes in locking & non-locking
Bullet Tipped Grasper
Grasping/Holding
- Atraumatic: common grasper used on most tissue types
- Names for its bullet shaped tip
Maryland Dissector
Grasping/Holding/Dissecting
- Atraumatic: used similarly as a debakey would be in a open case
- Fine curved tip is used for blunt dissection of fatty tissue
Aggressive Toothed Grasper
Grasping/Holding
- Traumatic: used to hold tissue that will be removed
- Commonly used to remove slippery gallbladder
Variable size (5/10mm)
Endoscopic Allis Clamp
Grasping/Holding
- Traumatic: To firmly hold moderate strength tissue or tissue being removed
Endoscopic Babcock
Grasping/Holding
- Atraumatic: Used for grasping fragile tissue
- Grasps bowel, appendix, & fallopian tube safely
Olson Clamp
Grasping/Holding
- Atraumatic: Used during a laparoscopic cholecystectomy in conjunction w/ taut catheter
- Once taut catheter is introduced into duct & advanced, clamp is locked shut, compressing duct
Endoscopic Biopsy Punch
Cutting
- Traumatic: used to remove a bit of tissue to be sent as a specimen to pathology
Blunt Probe
Retracting/Viewing/Probing
- Atraumatic: used to manipulate tissue while exploring the abd & pelvis
Needle Aspirator
Suctioning
- Traumatic: Device is used to collapse fluid filled cysts.
- needle end is used to puncture sac & liquid contents evacuated
Suction Bovie
Suctioning/Occluding
- will be attached to suction at its leur lock end
and to an electrical cord at metal prong. then
plugged into bovie machine & operated by surgeon
