What is encephalitis?
Acute inflammation of the brain parenchyma usually due to viral infection
Causes of encephalitis?
- Loads of causative viruses
- Measles
- Mumps
- Rubella
- Varicella zoster
- Enterovirus
- Adenovirus
Most common cause in UK of encephalitis?
Herpes simplex virus
Presnetation of encephalitis?
- Insidious onset
- Personality and behavioural changes
- Confusion and speech disturbance
- Reduced consciousness
- Coma
- Focal neurological deficits
- Seizures
Investigations for encephalitis?
- MRI
- EEG
- CSF
- Viral detection by CSF PCR can detect HSV or VZV
What MRI shows in encephalitis?
Areas of inflammation and swelling (bright areas)
Raised ICP
Midline shift may occur
What does an EEG of encephalitis?
Periodic sharp and slow wave complexes
CSF shows what in encephalitis?
Elevated lymphocyte count
Management of suspected VZV or HSV encephalitis?
Treated immediately with IV acyclovir even before investigation results are available
Long term complication of encephalitis?
Include:
- Memory impairment
- personality change
- Epilepsy
What is meningo-encephalitis?
Inflammation of both brain parenchyma and the meninges
