Mallory Weiss tear EM 1002
A
- hx of alcohol drinking and repeated vomiting leading to vomiting blood … = mallory weiss tear - all pt with upper GIT bleeding … should undergo risk assessment: 1. Blatchford assess. used for initial assessment for low risk pt who are candidate for outpt. R *** Urea 130/120, SBP >110, pulse 60, witnessed hematemesis(means continue bleeding), hemodynamic disturbance (pulse <100, SBP <110), known liver disease or varices, cardiac or malignancy

refer >5% in children and >10% in adults with partial thickness or more. IV fluids when >10% in children and >15% in adults.

Panic attacks
A
a sudden onset of intense fear which lasts maximum for 10 mins and resolves in the next 20 mins is almost always Panic attack…Fear of cardiac or pulmonary arrest is the hallmark of this conditio. In withdrawal or overdose scenarios stem will contain systemic signs like size of pupil, vital signs and often a history of drug usage( parties, needle marks)..

A
Calf swelling + Positive Homan’s sign (pain on dorsiflexion) –> Think of DVT even if no RFs.
