NSAID and HEART FAILUE

NSAID avoided in patient with Heart Failure / CKD / IHD
Selective COX2 inhibitors should also be avoided in these patients
Prostaglandins normally DILATE the afferent arteriole of the glomerulus and thus increase FILTRATION thus ↓pre load and ↑GFR .
NSAIDs inhibit Prostaglandins , your afferent arteriole will constrict, this will lower your filtration, and so Urine output will lower, and fluid will accumulate in the body, which exacerbates the heart failure.
