Clinical features of ectopic pregnancy (4)
Amenorrhoea
Vaginal bleeding
Lower abdominal pain
Shoulder pain
Signs on clinical examination (4)
Shock (hypotensive, tachycardic)
Peritonism (guarding, rebound tenderness, distension)
Adnexal mass
Cervical excitation
What is cervical excitation?
Significant pain upon moving the cervix in a pelvic exam. Also known as “chandelier sign”
How should a suspected ectopic pregnancy be investigated? (4)
Pregnancy test
Paired serum hCG
Ultrasound
Diagnostic laparoscopy
How are the results of paired serum hCG interpreted?
In a normal pregnancy hCG should double every day. Suboptimal in ectopic pregnancy (between 50% decline and 63% increase)
How is ectopic pregnancy treated? (3)
Salpingectomy/salpingotomy
Systemic methotrexate
If ruptured- laparotomy
